Réseau ACCESS Network aims to document and enhance the pathways to Hepatitis C Virus (HCV) care for individuals in jail and at the Supervised Consumption Site. The project involves capturing the experiences and workflows of nurses, case managers, and outreach teams. The goal is to create an engaging and informative toolkit that includes stories, pathway mapping, and patient quotes, making it a valuable resource for direct service providers. The toolkit will be built upon three years of data and program progression, incorporating best practices and examples from existing toolkits. The project will utilize Design Pickle and Canva Pro for graphic design needs.



Key objectives include:


- Documenting the pathways to HCV care.


- Capturing and presenting stories and quotes from service providers and patients.


- Creating a visually appealing and informative toolkit.


- Ensuring the toolkit is useful for direct service providers.

Deliverables

The project will result in a comprehensive toolkit that includes:


- A detailed pathway map for HCV care.


- Compelling stories and quotes from nurses, case managers, and patients.


- A visually engaging poster summarizing key findings and best practices.


- A well-designed toolkit using Design Pickle and Canva Pro, ready for distribution to service providers.
